Some years ago I got some very sound advice from a client. His advice was don't buy a dog if you intend to bark yourself. The basis was he had hired the firm for which I worked to design and manage the construction of a large power plant that was necessary to support a mining and refining complex his company was building in the Dominican Republic. He had come to my employer because he was told we were one of the best at what we did. He told us what he wanted done and we did it. I managed the project and when we met to review progress it was only to report we were doing what he had asked, within the time he wanted it and the budget we had agreed we could meet. Never once, even when we ran into snags, did he tells us how to do our job.
It appears Trump, unlike Ronald Reagan, has not picked people who could execute whatever vision he has, either because he hasn't effectively communicated to his subordinates what he wants or not enough so he could trust them to execute it.
The real problem is that the troops in the field have gotten used to new bosses nearly every four years and the boss level is too deep to start with. Its like a bank with dozens of vice presidents because under the law they must be officers in order to commit the bank to any deal, including signing up new depositors. The British solve this problem by staffing their departments with experienced professionals and the Ministers are merely figureheads. Nearly every one of their Ambassadors is a career professional.
The federal bureaucracy is the largest of its kind in the world. With its diverse responsibilities, most of which have been taken on despite the prohibitions in the Constitution, it is virtually unmanageable so it no wonder it is so inefficient.
Because our government has failed to tax to pay for all of the unconstitutional objectives it pursues its ultimate failure has been assured.
